AllThrive 365 to Continue Core Programs Previously Operated by Duet

PHOENIX — AllThrive 365 has assumed responsibility for three long-standing programs previously operated by Duet: Partners In Health & Aging, effective August 4, 2026.


The transition includes Duet’s Homebound Adult Services, Family Caregiver Services, and Grandparents Raising Grandchildren programs. According to the organizations, the three programs served approximately 3,000 Arizonans in 2025.


Founded in 1981, Duet has spent more than four decades supporting older adults, family caregivers, and grandfamilies throughout Arizona. Following Duet’s decision to sunset operations, the transfer was developed to preserve the services, relationships, and community trust established through that work.


AllThrive 365, formerly known as Foundation for Senior Living, has served Arizona since 1974 through programs focused on health, housing, caregiving support, transportation, and community connection.


Leaders from both organizations described the transition as a mission-aligned effort intended to maintain continuity and ensure that individuals and families who rely upon these services continue to receive support.


For the Arizona families, volunteers, and community partners connected to Duet, the announcement represents both the closing of an important organizational chapter and an effort to carry its work forward with care.
